THE FOLLOWING DOCUMENTS MUST BE COMPLETED AND SUBMITTED


By the inviting Professor:


• Formal request using the Visitors and Postdocs form on the School's website
• Full coordinates of invited postdoc
• Description of research project(s)
• Any special information to be included in the Letter of Invitation

By the Administrative Office:


• Letter of invitation (not required if the postdoc is a McGill student)
• Letter of agreement
• Appointment form (if paid by a McGill department)
• Award form (if receiving a stipend)

By the Postdoc:


• Postdoctoral Registration form
• Permanent Code Data form
• Personal Data form
• Letter confirming postdoctoral fellowship offer from agency or country
• Work Permit (if non-Canadian)
• Curriculum Vitae
• Ph.D. Diploma (or copy certified by a Commissioner of Oaths translated in English or French)
• Federal and Provincial Tax forms (if receiving a salary)
• Quebec Tax Exemption form (if receiving a salary)
• Copy of Birth Certificate if Canadian born.
• first ten items must be received no later than the expected date of arrival to avoid delays in funding
• Social Insurance Number card (if paid by a McGill Department)
• Medicare Card
• Direct deposit form (if paid by a McGill Department)

By the Postdoc:


• Postdoctoral Registration form
• Up-dated Quebec Medicare card
• Up-dated Social Insurance card (if paid by a McGill Department)
• Valid work permit

By the Administrative Office:


• Letter to extend the invitation
• Up-dated Letter of Agreement
• Appointment form (if paid by McGill department)
• Award for (if receiving a stipend)
• Copy of valid Work Permit
• Appointment form
